Since 2014, Foundation Communities has helped enroll nearly 30,000 individuals in health insurance under the Affordable Care Act.
Open All Year: Marketplace Insurance, Medicaid, CHIP, MAP
Our team of experts is available year-round to help you sign up for and use your health insurance. You must schedule an appointment, except during open enrollment. Walk-in clients are welcome only during Open Enrollment — November through December 15.
You may qualify for a Special Enrollment Period if you have a change in your tax household (e.g., get married or have a baby), a change in your immigration status, or a change in your health coverage (e.g., lose employer health insurance after losing a job). If you think you may qualify for one of these events, come see us today.
MAP assistance is available by appointment only.

-Big white envelope we gave you, if available
-Immigration documents for everyone applying for insurance, if applicable.
-Details and cost of health insurance offered by your/your spouse’s employer(s)
What to Know
-Username and password for healthcare.gov
-Current income information (recent pay stub, letter from Social Security, ledger)
-Names of doctors and medications for everyone applying for insurance
